I understand that participation in exercise and movement therapy classes involves physical activity and carries a risk of injury. I acknowledge that these classes are designed for people with Parkinson’s disease but may not be appropriate for everyone. I have consulted with my healthcare provider and discussed participation in exercise and movement therapy. I am responsible for monitoring my own condition and will notify the instructor and stop all activity that causes pain or dizziness. I fully assume all risks associated with my participation in this exercise program, including, but not limited to falls, injuries, or other medical events. I release and hold harmless my instructor, staff, volunteers, partners, and the Parkinson Association of the Carolinas from any and all liability, claims, injury, losses, or damages that may occur during or as a result of my participation in these classes. I agree to participate at my own risk.
